fill out the gift form and return it with your check made payable
to North Shore-Long Island Jewish Health System Foundation.
When we receive your gift, a card will be sent to the person(s)
or family named. The occasion, the person(s) honored, and, if
you wish, your message will appear on your card. The amount of
your gift will not be made known.
Your cancelled check will serve as a receipt for contribution
under $250. All gifts are tax-deductible according to law.
There are many ways to give. Your gift can be unrestricted or
designated to a specific program, center or division at Schneider

at (516) 465-2574 and charge your contribution to your Mastercard
or Visa account. A $25 minimum contribution is required when you
use your credit card. A card will be sent the day you place your
call.
