The Division of Adolescent medicine provides a very popular ACGME approved fellowship and training program in a wide variety of clinical settings. These include a hospital based out-patient clinic, two in-patient units, an office practice, an eating disorder program (including in-patient, out-patient, and day program components), off-site experiences in high school and junior high school health centers, two large group homes (serving youth with psychosocial problems) an HIV prevention program, and college health. The Fellowship program currently has five Fellows at all times and serves as the combined Fellowship for the Schneider Children's Hospital and North Shore University Hospital Fellowship programs, two of the first ten programs to be accredited in the United States in the field of Adolescent Medicine.

Fellows develop expertise in managing the medical, gynecologic, and mental health needs of adolescents in urban and suburban settings. Special emphases are placed on general medical care (including health screening, acute and chronic care), nutritional issues (especially eating disorders, lipids and obesity), gynecologic and sexuality-related issues (including pregnancy, contraception and STDs) and mental health care (including psychosocial management of complicated issues).

The fellowship is available for two or three years, with the major goal being a three-year fellowship which leads to eligibility for Board Certification in Adolescent Medicine. Fellows participate in teaching, research and administrative functions, in preparation for future careers in delivery of adolescent health care.

Division staff includes 7 attending physicians, 5 Fellows, 5 nurse practitioners, 3 nurses, 7 social workers, 2 psychologists, 3 nutritionists, and 3 health educators. Fellows are incorporated into the multidisciplinary approach practiced by the members of the Division in all clinical settings.

Division programs, though which Fellows rotate on a scheduled basis, include the following:

1) Eating Disorders - The Division provides in-patient, out-patient and day-program care for children, adolescents and young adults with anorexia nervosa, bulimia nervosa and related eating disorders. A multidisciplinary team, including Fellows, under the leadership of Drs. Neville Golden, Martin Fisher and I. Ronald Shenker, provides evaluation and management of medical, nutritional and psychological needs.

2) Hyperlipidermia and obesity - Children and adolescents with elevated cholesterol or triglyceride levels, as well as those with overweight or obesity, are evaluated and treated by Dr. Marc Jacobson, along with the fellows, nutritionists and nurses. Specialized diets, counseling, and medications as needed, are provided to patients and families in order to accomplish both short-term and long-term goals.

3) Adolescent Gynecology - Martha Arden, MD, Elba Iglesias, MD, and Linda Juszczak RN, PNP, PhD supervise Fellows in the provision of adolescent gynecologic services for a full range of needs. These include menstrual disorders (irregular menses, dysmenorrhea, dysfunctional uterine bleeding) and sexuality-related concerns (contraception, pregnancy, sexually transmitted infections). Confidential care is provided as needed.

4) Chronic fatigue syndrome - Dr. Martin Fisher, who has seen over 100 children and adolescents with the chronic fatigue syndrome, is available for evaluation and management. A comprehensive medical and psychosocial evaluation is performed for each patient, and treatment approaches, including cognitive-behavioral therapy, are provided as needed. Fellows participate with Dr. Fisher in this program.

5) Deaf health services - Dr. Eric Weiselberg is fluent in American Sign Language and is available to evaluate and provide primary care for deaf teenagers. He also has expertise in the evaluation and management of other handicapping conditions in adolescents. 

6) Behavioral Health - The Division is available to see adolescents with biobehavioral problems (such as those with substance abuse, attention deficit disorder, non-compliance with chronic illness care, depression/suicidal ideation) in which there is an interaction between medical and mental health issues. Evaluation, counseling, and coordination with mental health services are provided.

7) HIV Preventive Services - Under the leadership of LuAnn Skolnick, MSW, the Division runs a program funded by a grant from the New York State Department of Health. The program provides HIV education, counseling and testing for high-risk youth and works with the Division of Allergy/Immunology in providing medical and psychosocial care to HIV positive youth. Fellows have opportunities to participate in the clinical and research aspects of this program.

8) School-Based Health Services - The Division operates and provides care at three school-based health centers in Queens (Franklin K. Lane High School, Far Rockaway High School, Middle School #53) and two school districts in Nassau County (Sewanhaka, Bellmore-Merrick). Fellows develop clinical and leadership skills in school-based health at these settings under the supervision of Martha Arden, MD.

9) Group Home Health Care - The Division provides care to the high-risk youth at the St. Mary's (Syosset, Long Island) and St. Christopher Ottilie (Jamaica, Queens) group homes. Under supervision of the attending physicians, Fellows attend 1-2 sessions per week at these facilities.

10) Adolescent In-Patient Units - The Division of Adolescent Medicine provides on-going monthly Attending Rounds to the house staff and care to adolescent patients hospitalized in the in-patient units at Schneider Children's Hospital and North Shore University Hospital. Fellows participate in patient management and resident education on both units.
